REQUESTED RESPONSES:
The intent of this Request for Information is to establish sources in order to define the procurement strategy. Interested contractors are requested to respond in accordance with the following,
Please respond to this RFI if you are capable of providing the brand name or equivalent products listed in the Statement of Work found in the background section below.
POTENTIAL SOURCES SHALL PROVIDE ALL OF THE FOLLOWING INFORMATION IN THEIR RESPONSE:
1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large), proof that supplies are domestically made, and unique entity identifier used in SAM.gov.
2) Provide a Statement of Capability that demonstrates the offeror's past performance in providing these types of supplies. Please include the following: (a) Proof of OEM or OEM distributor of brand name supplies. (b) examples of prior completed Government contracts, references, and other related information. Please include contract number of previous contract(s).

BACKGROUND:

GENERAL SCOPE
This requirement includes healthcare furniture, furniture removal, professional installation and maintenance/warranty services. Healthcare furnishings are to include mobile wardrobes.
The contractor shall provide all labor, materials, equipment, transportation, and supervision necessary to satisfy the needs of each ordering activity.
All questions will be coordinated through the contracting officer. Product literature must be provided and cut sheets for each product detail. Late quotes will not be accepted. Furnishings shall have smooth finishes with no hazardous projections, sharp corners, and must be free of scratches and dents. The contractor shall track manufacturing schedules and notify the interior designer with any updates.
PERFORMANCE REQUIREMENTS
The contractor shall provide and be responsible for the technical assistance, development, and generation for final specifications.

1E remove offsite 32 wardrobes.
1B remove offsite 43 wardrobes.
3C remove offsite 40 wardrobes.

Furniture storage, delivery and installation will be required in/by December 6, 2022.
Duty hours will be 8:30 am 3:30 pm. The Government recognized US holidays are: New Year s Day, Martin Luther King, Jr. Day, Presidents Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day. If the holiday falls on Sunday, it is observed on Monday.
The contractor shall:
Coordinate and provide project management of all products and services to manage, design, order, ship, deliver and install new furniture from the manufacturer until final acceptance by COR.
Prepare and provide as built furniture drawings in PDF and AutoCAD.
Provide maintenance manuals for all products specified.

SUBMISSION TO RFI
The contractors shall submit complete emailed electronic copy portable document format (.pdf) or Microsoft Excel (.xls) format of:

Volume 1: (technical)
a. Submit a scaled drawing incorporating the products and services listed below. (PDF)
b. Submit a furniture schedule tagged to the floor plan.
c. Complete manufacturer s product specifications
d. Bill of Materials tagged per CLIN/ floorplan department and room etc. (no pricing)
e. Quantity of man hours for labor/installation
f. Project management plan
g. Product literature
h. Project staffing plan
i. Physical finish samples
j. Warranty.
k. Self-certifying statement confirming ability to meet project deadlines

Volume 2: (pricing)
a. Price quote with separate line for labor/installation
b. Bill of Materials with subtotals and tagged per CLIN/ floorplan

Responses to this RFI submitted by any other method will not be considered. All proprietary information shall be clearly marked. The use of hyperlinks is prohibited. Late submissions will not be accepted for evaluation.
PRODUCT
All finished surfaces shall be free of scratches, mars, dents, or blemishes, and withstand staining, and exhibit to flaking, cracking, or loss or adhesion. Furnishings shall have smooth finishes with no hazardous projections, sharp corners, or detail, which can be hazardous and cause personal injury or damage to clothing. See below for technical requirement of products.
SERVICES
Service required is detailed in General Requirements for Services and SV1 as outlined:

Design Contract Tasks/Requirements: The contractor shall be responsible for providing all samples.
The product must not be placed into production without clearance from the VA COR.
The contractor shall be responsible for taking and applying accurate field measurements to ordered product for verification of correct sizing. AutoCAD drawings may not be accurate finished dimensions and may not be used in place of field measurements. The Vendor shall be liable for any incorrect field measurements leading to incorrect product order.
The contractor shall provide final, clean 1/8 scaled drawings of product type for sign-off prior to scheduling manufacturing.
The contractor shall track manufacturing schedule and notify COR with updates of estimated completion date by email.

The Government maintains the option to modify the normal work week, days and hours, as necessary for the accomplishment of VHA mission.
